every item that drops with the reliquary comes with a custom message of the person that put that in the game. you can do that if you bought the most expensive supporter pack

Like they created the item? Or they selected that item to be a reliquary drop?
you select the item to drop. then you choose whatever message you want, and the foil variant. be aware that is not you that drop the item, you just select the item that goes into to the pool of available “selected uniques chosen by supporters”
you can choose the message and the foil effect variant
